About the RoleAs a CHSP Support Advisor, you'll work directly with clients in their homes, building trusted relationships, developing tailored support plans, and coordinating services that empower independence, wellbeing, and quality of life.
You will:
- Conduct home visits to assess needs and build individualised care plans
- Coordinate and monitor service delivery aligned with program goals and budgets
- Promote wellness, reablement, and capacity-building for clients and their carers
- Work collaboratively with internal teams and external stakeholders
- Make a real, tangible difference in people's lives every single day

- Excellent interpersonal and communication skills with a warm, approachable style
- Exceptional organisational and time management skills
- Proficiency in Microsoft 365 (Teams, Word, Excel)

- Eligible for $15,900 not-for-profit salary packaging plus a $2,600 meal entertainment card annually.
- Traineeship may be available to suitable applicants.
