About the Role

As a CHSP Support Advisor, you'll work directly with clients in their homes, building trusted relationships, developing tailored support plans, and coordinating services that empower independence, wellbeing, and quality of life.

You will:

  • Conduct home visits to assess needs and build individualised care plans
  • Coordinate and monitor service delivery aligned with program goals and budgets
  • Promote wellness, reablement, and capacity-building for clients and their carers
  • Work collaboratively with internal teams and external stakeholders
  • Make a real, tangible difference in people's lives every single day

About You
  • Certificate III in Aged Care or Community Services (or equivalent) is desirable
  • Ability to deliver person-centred, enablement-focused care.
  • Proven ability to meet performance targets while maintaining quality.
  • Current NSW Driver's Licence and safe driving record.
  • Solution-focused and proactive.
  • Builds positive client relationships and engagement.
  • Goal-oriented and motivated to help clients achieve outcomes
  • Commercial awareness, including service pricing.
  • Strong computer literacy with the ability to work across multiple client management systems
  • Excellent interpersonal and communication skills with a warm, approachable style
  • Exceptional organisational and time management skills
  • Proficiency in Microsoft 365 (Teams, Word, Excel)
