About The Role

The Program Coordinator / Team Leader plays a vital leadership role in managing day-to-day operations across our disability support programs. You'll oversee staff coordination, participant support alignment, program planning, incident management, and rostering—ensuring smooth, person-centred service delivery across all sites.

You'll work closely with the Program Director to align daily operations with Yahweh Care's mission and values.

Key Responsibilities

  • Lead and coordinate frontline support workers across multiple programs
  • Manage daily program operations, ensuring effective service delivery
  • Create and manage staff rosters to meet client needs
  • Plan and monitor individualised client programs and activity schedules
  • Align staff skills and availability with participant needs
  • Provide leadership and coaching to support workers
  • Review and submit incident reports and follow up on actions
  • Maintain high standards of care and compliance with NDIS and Yahweh policies
  • Collaborate with internal and external stakeholders, including families and allied health providers

What We're Looking For
  • Minimum 3 years' experience in a disability, aged care, or community services environment
  • Prior experience in team leadership or coordination (essential)
  • Certificate IV or higher in Disability, Community Services, or related field
  • Strong organisational and time management skills
  • Excellent communication and problem-solving abilities
  • Sound knowledge of NDIS standards and compliance
  • Proficient with rostering systems and incident reporting platforms
  • Current First Aid & CPR Certificate
  • NDIS Worker Screening Check & Orientation Module
  • Working With Children Check & National Police Check
  • Driver's licence and reliable vehicle
