Join Our Team as a Community Support Worker – Support Independence, Your Way

The Eyre and Far North Local Health Network is embracing a Consumer Directed Care model for delivering aged care, disability, and support services—placing control directly in the hands of consumers. This model allows individuals to choose their services, timing, delivery methods, and providers.

As a Community Support Worker, you will assist people who are aged, frail, or living with disability to maintain their independence, whether at home or in residential care. Responsibilities include personal care, cleaning, meal prep, transportation, and social support, tailored to individual needs and goals.

You will collaborate with consumers, families, and health professionals to support quality outcomes and promote dignity and independence. Success in this role requires flexibility, empathy, self-motivation, and confidence working independently in the community.

What We're Looking For :

  • Current driver's license and willingness to drive.
  • Clear Criminal History Check.
  • Ability to be contacted by phone.
  • Access to a reliable, registered, and insured vehicle (minimum third-party insurance).
  • Meet immunisation requirements as per the Immunisation Guidelines for Health Care Workers in South Australia 2014.

Applicant Information

This is a part-time position, working 40 hours per fortnight, with salary adjusted accordingly. We promote diversity and flexible working arrangements. Appointment is contingent on a satisfactory Criminal History Check and immunisation compliance.

  • DHS NDIS Worker Check
  • National Police Certificate for unsupervised contact with vulnerable groups

Immunisation risk category: A (direct contact with blood or body substances). SA Health implements workplace immunisation requirements.
