Community Support Worker

Are you passionate about empowering individuals to take control of their care and live independently? We're seeking a dedicated Community Support Worker to provide personalised support within the Consumer Directed Care model at Eyre and Far North LHN.
In this role, you will assist aged, frail, or disabled clients with personal care, medication support, cleaning, meal preparation, transport, and social support. You'll work closely with consumers who actively choose the services they receive, how often, and who provides them—helping them maintain comfort and independence in their own homes.

What We're Looking For:- Completed Certificate III or higher in Individual Support (Aged Care, Disability, or Home and Community)
- Proven experience in exercising judgment and initiative in the day-to-day execution of duties
- Experience in providing direct care services in a health-related field
- Experience working with aged, frail, or disabled persons
- Proficiency in computer packages e.g., Microsoft Word, Excel

Checks- DHS National Disability Insurance Scheme (NDIS) Worker Check
- National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups (e.g., aged care or vulnerable clients)
Immunisation risk category: A (direct contact with blood or body substances).
SA Health Services are required to implement workplace vaccination policies.
