For over 15 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.

Duties and responsibilities may include :

• Travelling between customer's homes and providing individual and personalised support to customers in accordance with their support plans and / or medical practitioner's directions.
• Working with or under the guidance of Registered Nurses, Allied Health professionals and medical practitioners
• Medication Administration and Complex Clinical Care e. Palliative Care, Diabetes, Bowel Care / Suppositories, Catheter Care, Stoma Care, Oxygen and more
• Monitoring and communicating on the customer's wellbeing and reporting on progress of customers
• Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
• Provide coaching and guidance to Community Support Professionals and Support Staff / Case Managers as required
• Demonstrating a commitment to best practice and evidence of continued professional development
• Participating in staff meetings and attending training opportunities when required
• Personal care : showering, dressing, grooming and medication assistance
• Domestic services : light household cleaning, laundry and meal preparation
• Social support, social activities, shopping / groceries, travelling to medical appointments
• Supporting customers in the use of aids and personal equipment including : standing or walking frames, wheelchairs, manual handling or electric hoists, sliding sheets or moving boards, hearing aids and other physical aids
• Supporting participants within Just Better Care's Supported Independent Living Home
• Supporting customers with complex and / or challenging needs
• Monitoring the well-being of customers and reporting on their progress

What you will need :

Minimum Diploma of NursingCurrent CPR / First Aid CertificateCurrent and satisfactory Criminal Record CheckCurrent and satisfactory Federal Police Check, NDIS Workers Screening Card & Blue CardAHPRA RegistrationCurrent Australian Driver's LicenceReliable, registered and insured vehicle (comprehensive insurance is preferred)Smartphone with internet accessProof of of Vaccination (where relevant) i.e. COVID-19 vaccination certificatePrevious experience in the community care or health / aged care sectorUnderstanding of DVA Community Nursing guidelines or previous community nursing experienceCert IV in Workplace Training & Assessment
