What You'll Do:

In this dynamic and rewarding role, you'll:

  • Partner with participants to design and implementpersonalised, strengths-based NDIS plans.
  • Connect people with the services and supports that alignwith their goals and needs.
  • Use recovery-oriented and person-centred approaches tobuild skills and confidence.
  • Coordinate service delivery, monitor progress, and adaptplans as required.
  • Collaborate with a multi-disciplinary team and contributeto service innovation.
  • Maintain accurate records and ensure alignment with NDISpractice standards.

What You'll Bring:

  • Significant experience in the disability or human servicessector.
  • Knowledge of intervention models and ability to coordinateservices.
  • Specialist knowledge of NDIS and related supportservices.
  • Strong problem-solving and conflict-resolution skills.
  • Ability to advocate effectively for participants andpromote their independence.

Your Toolkit:

  • Tertiary qualification in community services, social workor equivalent and extensive community work and training(essential)
  • Current SA driver's licence (essential)
  • Current Safe Environments for Children and Young People– Through Their Eyes Certificate (essential)
  • Current DHS Working with Children Check (essential)
