About the facility
The Chronic and Complex Care Program is a large multidisciplinary, community based program employing nurses, allied health (incl. social workers, physiotherapists, occupational therapists, psychologists, dietitians, pharmacy, neuropsychologist) and medical staff (incl. Gen Med and Psychiatry), that provide assertive clinic based and outreach support for people with chronic health and complex psychosocial needs.

The Chronic and Complex Care Program aims to decrease hospital demand through the provision of comprehensive assessment, care planning and care coordination, in addition to facilitating timely access to specialist care, for people who are frequently, or are at imminent risk of presenting, to hospital.


About the role


The Chronic and Complex Care Program are looking for an experienced nurse or allied health professional to join our team.

Position will initially be located at a location in the Northern Suburbs of Geelong, with an option of rotation in the future.
