  • New positions created to expand services and establish a community intensive treatment team (CITT) at our Corio adult continuing care team
  • Utilise your skills to target and treat a vulnerable population in the community
  • Multiple positions available permanent full time and permanent part time

About the service


Barwon Health's Adult Continuing Care Services (CCS) provides community supports to people aged between 16 and 65 years, experiencing serious mental health needs.

Our service delivery includes comprehensive crisis assessment, crisis management and treatment, case management of complex issues and mobile outreach.

We have community case teams situated in five locations across the Barwon region ranging from Corio and Geelong West to Bellarine, Torquay & Colac.

Each team is situated in a community health centre that is close to the city centres, beaches or local community hubs.


CITT
focuses on providing intensive care and treatment for the most severely ill group of individuals with mental illness in the community.

In particular, those that are hard to engage, at risk of hospitalisation, and those that require high intensity psychosocial input.

CITT aims to ensure continuity of care, to prevent admissions, and to stimulate inclusion, so that individuals can participate in society.


About you

  • Formal professionally recognised qualifications in Psychiatric Nursing, Occupational Therapy, Social Work or Psychology, and APRHA registration where required
  • A demonstrated high level of clinical skills in the assessment, treatment and continuing care of adults with mental health disorders in a community setting
  • The ability to maintain effective working relationships and clinical service delivery in an environment which is characterised by competing demands
  • Thorough knowledge of the objectives and provisions of the Mental Health Act and amendments

As the successful applicant, you will provide assessment, treatment and recovery based interventions to individuals and their families who experience serious mental illness.
