The Barwon Post-Acute Care Program (BPAC) is under the Chronic and Complex Program portfolio, that aims to support a safe and timely discharge for people in public health bed based services, assessed as requiring short-term, community based support and coordination to aid recuperation in the community setting.

BPAC Clinicians also support the triage and allocation of Community Nursing and CHRP referrals submitted via BOSSnet

About the role


The Barwon Post-Acute Care Program have two part-time positions available Registered Nurses (Division 1) to work 32 hours a fortnight.

We require these positions to have the ability to pick up extra shifts to cover planned and unplanned leave, over a 7 day a week roster.


As a Barwon Post-Acute Care Program (BPAC) Clinician, you will support a team of health professional to triage, assess and broker services to support clients requiring short-term support following a bed-based admission at a public bed based health care.

The role requires clinicians to work collaboratively with acute, subacute and community based services.

The BPAC team is located at 120 Settlement Rd Belmont with HARP, Remote Patient Monitoring, HealthLinks, Information and Access and the Social Support Program.

The role is largely office based, working with clients over the phone or via telehealth.
