About Our Facility

Barwon Health's Connected Care Program and Hospital Admission Risk Program (HARP) are community-based services employing nurses, allied health (incl. social worker, physiotherapists, exercise physiologists, dietitians, occupational therapy, psychology, neuropsychology) and medical staff that provide assertive outreach and clinic based support for people with chronic health and/or complex psychosocial needs.

Both the Connected Care Program and HARP aim to decrease hospital demand through the provision of comprehensive assessment, care planning and care coordination, and addition to supporting timely access to specialist care, for people who are frequently, or are at imminent risk of present to hospital.


About the Role


The Barwon Connected Care Program and HARP are looking for experienced nurses or allied health professionals to join their team.

The position will be located at either the Northern Suburbs, and/or at Belmont, Geelong, which can be discussed at interview.
