We are looking for an experienced Aged Care Coordinator to become part of our dynamic and collaborative team.

The ideal candidate must be a team player with exceptional interpersonal communication skills, outstanding organizational skills, high motivation, and a dedication to working with senior citizens.

This role is fast-paced and would be best suited for someone with prior experience in a similar coordination position, particularly within the Community Aged Care Industry. We welcome applications from Enrolled Nurses. The primary focus of this role is on case management.

The Role Offers:

  • Maintaining client records and ensuring compliance with organizational and quality standards
  • Conducting various client assessments and managing documentation
  • General administrative tasks such as email monitoring, data entry, scanning, and filing
  • Delivering exceptional customer service to clients, staff, and stakeholders
  • Managing clients' budgets according to government funding
  • Emphasis on teamwork and quality assurance

Working Schedule:

  • Monday to Friday, typically 8.30 am - 4.30 pm
  • Job Type: Permanent Part-time, 37.5 hours/week
  • Availability for on-call duties on rotation
  • Competitive salary with reimbursement for kilometers traveled using personal vehicle

Requirements:

  • Minimum 1 year of experience in aged care or community care, with knowledge of government-funded home care packages
  • Excellent verbal and written communication abilities
  • Confidence, compassion, enthusiasm, a friendly demeanor, and a positive attitude
Qualifications
  • Certificate III in Aged Care/Individual Support/Community Care - mandatory
  • Australian Driver's License
  • Reliable vehicle with comprehensive insurance coverage
  • Current National Criminal History Check
  • Current First Aid and CPR Certification
  • Blue Card (or willingness to obtain one)
Preferred Skills
  • At least 1 year of experience in a similar role highly preferred
  • Familiarity with government-funded home care packages
  • Proficiency in the Microsoft Suite and other computer applications
  • Confidence, competence, and a proactive approach to tasks
  • Demonstrated ability to manage time effectively, plan, and prioritize tasks independently
  • Strong attention to detail
  • Excellent verbal and written communication skills
