Summary
Reporting to the Program Manager for Home Support, the
Care Manager will be responsible for managing allocated cases within Home Support Services including the management and supervision of Home Care Employees (HCEs), entering services into the scheduling system, managing invoices, and liaising with other services providing care and assistance to clients of the service.
Key Responsibilities
- You will be a primary point of contact for clients and carers (HCEs)
- Undertaking a collaborative and goalorientated approach to developing care plans.
- Maintain oversight over clients' outcomes and changes in circumstances.
- Contribute to and implement quality improvement and risk management into all aspects of service.
- Work with customers to recognise existing and build new capabilities to support inhome services journey.
- Leverage your knowledge in the home care sector and the broader aged care system to support your customers through action and experience.
- Providing supervision and clinical support to (HCEs)
- Identifying and providing training and education programs for professional development to team.
- Promote a culture of best practice and shared knowledge by sharing your experience in the home care sector and the broader aged care system, supporting employees and clients through action and experience.
- Uphold and delivery of services in accordance with the Aged Care Act and Aged Care Quality Standards
Key to the success in this role is the demonstrated ability in the below skills and experience:
- Tertiary qualifications in Social Work, Social Science, Allied Health or Nursing.
- Demonstrated experience in Home and Community Care, ideally with Aged care experience.
- Welldeveloped understanding and delivery of clinical support and/or case management experience.
- Current Australian Full Drivers Licence and access to your own comprehensively insured vehicle.
- Strong communication skills and customer service focus.
- Knowledge of Health or Community Aged Care Services standards or Home Care (desirable).
