Due to the increased business demands, this dedicated NDIS service provider is looking for an experienced
Support Coordinator to join their team based in their Pakenham office.


About the Role


You'll use your expertise to assist NDIS participants in the coordination of their support funded NDIS plan, including monitoring and implementation of all supports in their funded plan - such as informal, mainstream and community supports.

Using your professional skills and knowledge, you will assist clients to develop attainable goals and, in turn, coordinate a range of services that help them regain control and a quality of life.


We're looking for someone who can maintain a high level of professionalism and work as a dependable member of the team, is personable by nature, and is a witty character who can bring a light hearted atmosphere for the clients.


Roles and responsibilities

  • Providing Coordination of Supports to complex programs in allocated portfolio.
  • Contribute towards participant assessment care planning, response to referrals and reviews.
  • Support the individual's ability to connect with informal, mainstream, and funded supports.
  • Ensuring the quality of support is in line with best practice and in line with allied health partner, Support Coordinator / Case Manager recommendations.
  • Provide crisis management and Incident reporting follow up.
  • Facilitating regular team meetings and attending care team meetings for client programs in allocated portfolio.
  • Completion of critical incident review processes, including participating in and documenting investigations as required.
  • Advocate internally and externally to ensure systems and services protects the rights of the participants and give greatest participant control over decision making
  • Contribute to continuous improvement culture that reviews indicators of participant satisfaction and implement action to achieve results
  • Support service recipient to understand their NDIS plan, goals, and objectives

Skills and Qualifications

  • 1 to 2 years prior experience as a support coordinator for NDIS Packages
  • Certificate III in Disabilities Services, Aged Care or Bachelor degree in a related field
  • Experience working with people living with a disability & in coordinating programs for people with a disability
  • Knowledge of the NDIS Operational Guidelines and NDIS Quality Standards, including a detailed understanding of case management practices and principles.
  • Welldeveloped communication (both oral and written) skills and interpersonal skills
  • Excellent documentation and computer skills
  • Welldeveloped work organisation skills, the ability to manage time effectively and prioritise tasks.
  • Ability to liaise and consult with a wide range of clients, health professionals and team members
  • NDIS Screening check.
  • Own registered and insured vehicle with a current driver license
  • Working with Children Check.
  • Police Check
  • Full Working Rights in Australia
