How you will make an impact

The Registered Nurse will work collaboratively in the process of assessment, planning and implementation of services which meet a patient's health needs. Registered nurses will also be responsible for conducting outreach nursing, primarily wound care and intravenous antibiotics to patients in their own homes.

They will act as an active resource to Cabrini's Ambulatory Services to internal and external health providers in matters pertaining to patient care coordination and access to Cabrini's Sub acute Services, within the framework of the Mission, Values and Vision of Cabrini Health.

Cabrini's Hospital in the Home Unit has a casual position available for a skilled registered nurse with an interest in home based nursing. This is a rewarding role as you will provide acute care to support patients in their own homes.

Key responsibilities include:

  • Conducts a comprehensive plan, assessment and implementation to all patients' in their own home.
  • Consults with the patient, their family and/or carers, the patient's medical staff, nursing staff and other members of the healthcare team regarding the current medical needs.
  • Report any changes in patient status to relevant staff and personnel to ensure proper treatment is implemented
  • Works collaboratively with all departments to ensure best practice is implemented to patients in the home.
  • Ability to work autonomously and as part of the team

You will have:

  • Current registration as a Registered Nurse with AHPRA.
  • Valid driver's licence
  • Current Working with Children Check
  • Proven experience working within the acute and community nursing

Key Capabilities:

  • Comprehensive knowledge of wound care/VAC therapy
  • Intravenous catheter cannulation competency
  • Management of venous access devices and administration of medications
  • Chemotherapy experience (desirable) but not essential
