About the Role:
mecwacare's Home Nursing and Care Services is seeking an experienced community nurse to provide home-based nursing care to our clients. The successful applicant will provide holistic and evidence-based nursing to a variety of clients of all ages for post-acute care (PAC) or hospital in the home (HITH) patients.
Key Responsibilities:
- Provide home-based nursing care to reduce hospital stays and improve long-term patient health.
- Assess and develop individualized care plans for clients.
- Deliver high-quality nursing care, including wound care, IDC or stoma care, personal care, medication administration and supervision, blood glucose monitoring, and application of compression stockings.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
- Communicate effectively with clients, families, and healthcare professionals.
Requirements:
- Registered Nurse grade 2 with the Australian Health Practitioner Regulation Agency (AHPRA).
- Minimum 5 years of community nursing experience.
- Strong initiative and decision-making skills.
- Effective time management and problem-solving abilities.
- Current driver's license and registered vehicle.
- Current NDIS Clearance.
