The successful applicant will work with a variety of clients of all ages, providing post-acute care (PAC) or hospital in the home (HITH) services. These services may include wound care, IDC or stoma care, personal care, medication administration and supervision, blood glucose monitoring, and application of compression stockings.
Key Responsibilities:
* Provide high-quality, patient-centered care to clients in their homes
* Work collaboratively with the multidisciplinary team to develop and implement individualized care plans
* Conduct thorough assessments and develop care plans that address the physical, emotional, and social needs of clients
* Provide education and support to clients and their families to promote independence and self-management
* Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements:
* Registered Nurse grade 2 with the Australian Health Practitioner Regulation Agency (AHPRA)
* Minimum 5 years of community nursing experience
* Strong initiative and decision-making skills
* Effective time management and problem-solving abilities
* Current driver's license and registered vehicle
* Current NDIS Clearance
