What You Will Be Doing
The Chronic Disease Monitoring Service Team Leader is a valued member of the Community Health Leadership Team, committed to upholding Peninsula Health's values and meeting organisational goals.
Each participant will be assessed and offered a suitable device/s, depending on their digital literacy and the severity of their condition.
Each participant will be monitored remotely by the monitoring service team on a daily basis.The Team Leader position will manage a team of Registered Nurses and Allied Health staff, with Medical support.
The Team Leader will collaborate with key stakeholders to implement the new model of care considering; clinical risk, inclusion criteria, intake processes and clinical escalation processes.
- Implementation of monitoring service agreed model of care in conjunction with the Program Manager and task group
- Undertake recruitment and selection of delegated staff
- Define roles and responsibilities within the team, with clear escalation pathways developed
- Provide line management, supervision, support and coaching for monitoring clinicians
- Work in partnership with other clinicians and managers to ensure the model is integrated with other services and streamline the journey for patients
