Aged Care Assessment Team Assessor

Where you'll be working:

The Aged Care Services Team is located in multiple centres across the Hunter including Newcastle, Lake Macquarie, Port Stephens, and Lower Hunter.


The CACs GNS Aged Care Services comprises of ACAT, Dementia Advisory Service, Regional Assessment Service, Community Dementia Nurses, Neuropsychology Service, and Hunter Carer Support.


About the role:

  • You will be responsible for working together with older people, their families, and carers to identify their goals and wishes through completing a comprehensive assessment, aged care approvals, and facilitating timely referrals to Commonwealth Home Support Programfunded services and health services
  • You will work collaboratively within a multidisciplinary team and with Geriatricians, GP's and Practice Nurses, other Health Services, and Non-Government service providers
  • An extensive orientation will be provided including National ACAT training to access My Aged Care orientation and the initial education period will take approximately 2 months and will be based at Aged Care Services, Booth Building, Longworth Avenue, Wallsend NSW 2287 for this period
  • After the initial training period, you will then be advised which location will become your accustomed place of work
  • The service locations are any of the following: Wallsend Campus, Rankin Park Campus, Newcastle Community Centre, Westlakes Community Centre (Toronto), Eastlakes Community Centre (Windale), Nelson Bay, Raymond Terrace or Kurri Kurri Community Centre

Requirements:

  • Must hold a qualification in one of the following: Occupational Therapy, Nursing, Social Work, which provides eligibility or membership of the relevant professional association / provisional or general registration through the Australian Health Practitioner Regulation Agency (AHPRA)
  • Eligibility to drive in NSW including willingness to travel to conduct home visit assessments

  • Experienced clinician with knowledge of Aged Care and the Aged Care Community System
  • Advanced computer skills
  • Advanced assessment and problemsolving skills
  • Ability to work independently and within a multidisciplinary team
  • Welldeveloped time management skills
  • Demonstrate excellent written and verbal communication skills
  • Understanding and provision of personcentred care in collaboration with clients, carers, and service providers to achieve identified outcomes

Acceptable proof of COVID-19 vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement (IHS), or AIR COVID-19 Digital Certificate, or AIR COVID-19 and Influenza IHS.

Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
