About the service:

The Head to Health service is a new service for adults in Western Sydney seeking emotional wellbeing and mental health support. The service provides a combination of peer-led recovery and clinical support and a safe, welcoming and supportive environment for people experiencing psychological distress. Allowing low risk individuals to identify alternatives to presenting to the Emergency Department and gaining access support from trained mental health professional within their community, outside a clinical setting.

Role Overview:

This is an exciting opportunity to be part of a new Head to Health service in Western Sydney.

  • Participate in intake system, answering phone calls and enquiries, screening and assessing needs, prioritising and making appropriate follow up appointments and referrals
  • Conduct bio-psycho-social assessments of individuals presenting to the service, using the Stride Head to Health Hub assessment tool and document the results of assessment and screening
  • Conduct risk assessments, including assessment of suicide risk, violence/aggression and vulnerability and provide risk management in collaboration with the individual, their family/carers and other support systems with active engagement of appropriate referrals and treatment planning
  • Liaise with and develop collaborative working relations with key service providers
  • Provide brief interventions to a small caseload of clients with mild to moderate mental health presentations as required

Requirements:

  • Must have one of the following: General/Provisional AHPRA registration as a Psychologist OR General AHPRA registration as an Occupational Therapist or Mental Health Nurse OR AASW eligibility as a Social Worker
  • Experience in mental health assessment and subsequent coordination of access to appropriate service provision
  • Knowledge of and experience in, the provision of mental health services to individuals, and their families, including knowledge of contemporary evidence-based interventions
  • Ability to develop and deliver individual or group-based skills training activities
  • Highly developed communication (including written) and interpersonal skills, and the ability to engage with individuals, advocate on their behalf and establish and maintain effective relationships with staff, clients, carers and stakeholders
  • Good computer skills
  • Must be eligible to apply/currently hold a WWCC
  • Must hold/be willing to apply for NDIS worker screening clearance
  • Current Drivers License
