Clinical Care Coordinator – Casual

Location: Victorian Pride Centre, St Kilda, VIC
Program: Post-Acute Care
Division: Client Services
Classification: Community Health Nurse Grade / Social Worker Grade 2 / Health Professional Grade 2
Award: Nurses and Midwives (Victorian Public Sector) Enterprise Agreement 2020-2024

The Clinical Care Coordinator will play a key role in the Post-Acute Care program, supporting clients discharged from public hospitals by coordinating short-term, community-based care. You will collaborate with internal teams and external partners to ensure safe, timely discharges and provide high-quality care coordination.


Key Responsibilities

  • Conduct timely client assessments and implement care packages based on individual needs and goals.
  • Coordinate activities with hospital teams, clients, families, and community stakeholders.
  • Broker services and oversee care delivery during the Post-Acute Care period.
  • Ensure clients are linked to ongoing community supports where necessary.
  • Maintain accurate records and contribute to quality improvement initiatives.


What We’re Looking For

  • Qualifications: Degree in Nursing, Social Work, or other Allied Health disciplines.
  • Experience: Minimum 3 years’ experience in both hospital and community service settings, with knowledge of Post-Acute Care or similar programs preferred.
  • Skills: Excellent interpersonal, coordination, and report-writing skills, with proficiency in Microsoft Office and databases (preferably Fixus).
  • Ability to work flexibly in a multidisciplinary team, adapting to changing priorities.
