Clinical Care Manager Role Overview

HammondCare At Home is seeking a skilled Registered Nurse to fill the role of Clinical Care Manager. This is a rewarding opportunity for an RN to take on a leadership position and make a real difference in the lives of our clients.

Key Responsibilities:

  • Manage the clinical care of clients in the Northern Sydney Region, working closely with our team to ensure the highest level of care and support.
  • Responsible for the clinical assessment of each Home Care Package client, providing ongoing clinical support and advice as needed.
  • Support with COVID escalations and care worker education, ensuring our team is equipped to provide the best possible care.
  • Lead, support, and mentor our team of professionals, promoting a culture of excellence and collaboration.

Requirements:

  • Registered Nurse with a current AHPRA clinical qualification.
  • At least 2 years of experience working as a Registered Nurse, with a strong focus on wound management.
  • Strong knowledge of Home Care packages and experience working in a multisite environment.
  • Willingness to travel between sites and hold a current Australian licence and access to their own car.
