The key focus of the role of Team Leader is to provide leadership and mentoring for Community Aged Care Care Managers including;

  • Ensuring best practice high quality client care services;
  • CDC budget management and overseeing care planning in general;
  • Ensuring all expenditure is monitored against allocated budget for clients and claims conference;
  • Supervision of  home care support workers;
  • In collaboration with the L&D Manager, Identifying training opportunities for Home Care Support Staff;
  • Ensure mechanisms are in place for reviewing case assessments and care plans on a regular basis;
  • Monitoring the performance of Care Managers and providing guidance and direction where appropriate to ensure quality and services meet customer needs;
  • Providing case co-ordination and supervision;
  • Monitoring workflow and assisting in establishing effective schedules and routines;
  • Monitoring adherence to legislation, program guideline, procedures under which services are delivered, identifying specific skills training and coaching;

Working closely with the Operations Manager CAS, the Team Leader will assist in the allocation of clients to Care Managers and all internal auditing relating to CDC operations and CDC packages managed by JewishCare.

Successful candidates will demonstrate the following;

  • Experience in aged care and ACQSC standards, ideally with a knowledge of CDC programs;
  • Proven ability to assess client support needs holistically and develop care plans that support those needs;
  • Demonstrated experience in coaching and mentoring, supervision and development of staff;
  • Strong stakeholder management skills along with the ability to medicate difficult situations and crises with consumers and service providers;
  • Tertiary Qualifications in or a minimum of a certificate in Individual Support or Aged Care;
  • Ability to advocate and liaise professionally with internal and external stakeholders;
  • Proven experience investigating professional/technical knowledge requirements applied to the teams’ work processes;
  • Ability to mediate difficult situations and crises with consumers and service providers;
  • Demonstrated Critical thinking;
  • Proven Problem Solving skills;
  • Demonstrated computer skills using CRM’s and Microsoft Suite;

Highly desirable

Clinical qualifications in related Aged Care fields such as Dementia.
