As a Team Leader, you will be responsible for providing leadership and mentoring to a team of Care Managers, ensuring high-quality client care services and overseeing care planning and budget management.

Key Responsibilities
  • Leadership and Mentoring: Provide guidance and direction to Care Managers to ensure quality and services meet customer needs.
  • Care Planning and Budget Management: Oversee care planning and budget management for clients and claims conferences.
  • Supervision and Development: Supervise home care support workers and identify training opportunities for staff.
  • Case Coordination and Supervision: Provide case coordination and supervision to ensure effective schedules and routines.
  • Legislative Compliance: Monitor adherence to legislation, program guidelines, and procedures under which services are delivered.
Requirements
  • Experience: Proven experience in aged care and ACQSC standards, ideally with knowledge of CDC programs.
  • Qualifications: Tertiary qualifications in or a minimum of a certificate in Individual Support or Aged Care.
  • Skills: Strong stakeholder management skills, ability to mediate difficult situations, and demonstrated critical thinking and problem-solving skills.
Desirable Qualifications
  • Clinical Qualifications: Clinical qualifications in related Aged Care fields such as Dementia.
