About the opportunity

You will be part of the Community Plus Team. This is a recovery focused multi-disciplinary team who provides an extended hours clinical service to people who live within the municipalities of Wyndham, Hobson's Bay and Maribyrnong. (It can be likened to a combined MSTS and HOPS program). In addition to experiencing significant mental illness/ mental health problems your clients will have other health and psycho social needs that significantly impact their wellbeing

  • You will provide clinical case management to a small number of clients, often engaging clients via outreach

  • You will work with your clients, their carers and other service providers to develop and facilitate comprehensive treatment plans.

  • You will also support and be supported by your Community Plus colleagues.

  • While most shifts will be day shifts Mon-Fri you will be rostered across a 7 day roster, being assigned some weekday evening shifts and weekend / public holiday day shifts.

  • Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.

  • A compassionate, flexible and positive approach to effective patient care and management.

  • The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.

  • Experienced in managing a caseload and understanding interventions and services that support clients with complex interconnected needs
  • A willingness to provide support and supervision to junior staff and students
  • Registration with AHPRA as a Clinical Psychiatric Nurse, Occupational Therapist or Psychologist with endorsement as a Clinical Psychologist. Or you are a Social Worker eligible for registration with AASW.
  • Have a full Australian Drivers License (not P Plates)

You will also be required to provide evidence of, or in the process of obtaining:

  • A current Police Record Check and Working with Children Check.

  • Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.

  • Current Australian full Driver's license
