Sylvanvale Limited is an award winning, not-for-profit service provider that supports over 750 people with disability. We pride ourselves on assisting individuals, from a range of diverse cultural backgrounds, to live and participate in their community as they choose. Our services are provided at sites across greater Sydney including the Blue Mountains.
About the service
The Individual Apartment Living service is an identified area for growth at Sylvanvale and this new model of support currently includes 3 existing locations in the St George and Sutherland Shire areas.
Each location has its’ own Individual Apartment Living team which provides drop-in support to ten individuals with a disability living in their own apartments in a residential unit block. Each location offers brand new, luxury apartments to the tenants.
To support the tenants to live independently, we partner with disability housing providers who ensure the tenants are supplied with the required Assistive Technology and Sylvanvale provides the tenants the requested level of support through our innovative service.
About the role
- Based in Kogarah.
- Monday to Friday, full time, shifts between 6.00am and 4.00pm. No on call, weekend or public holiday hours.
- Hybrid mix of hours on the floor providing direct support to our clients, as well as dedicated hours to complete Team Leader administrative duties. A great opportunity to mentor staff and manage client feedback in real time

Reporting to the Regional Manager you will utilise your strong coaching and mentoring skills to support a team of Disability Support Workers develop and implement best practice.
Responsibilities include:
- Maintaining knowledge of NDIS Practice Standards and NDIS Operational Guidelines.
- Ensuring high quality, client-centred support is provided by Disability Support Workers.
- Facilitating support and supervision of frontline staff and monthly Team Meetings.
- Being the key point of contact for clients and frontline staff.
- Ensuring all client documentation is current and staff are trained in implementing support that is specific to their needs.
- Ensuring frontline rostered hours do not exceed those consented to in client Service Agreements.

You are constantly striving for continuous improvement and delivering high quality services. You are a great communicator and invest in developing great stakeholder relationships.
Essential:
• Customer driven approach to service delivery.
• Ability to achieve results and deliver on quality.
• Ability to identify trends and drive process improvement.
• Solid understanding of the NDIS Quality and Safeguarding Framework and Practice Standards.
• Knowledge of providing services and support to people living with disability.
• Working knowledge of budget management.
• Solid report writing skills and data analysis.
• Strong stakeholder engagement skills.
Desirable:
• Certificate IV in Disability or similar.
Successful applicants will require a current Australian Drivers Licence and First Aid certificate and a full COVID-19 vaccination and booster. An offer will be subject to a police clearance, a pre employment medical and a NDIS Worker Screening Check.
