About the Role

We are seeking an experienced Independent Living Support Specialist to join our team and play a vital role in supporting elderly individuals in their communities.

This is a permanent, full-time opportunity that requires you to work autonomously from home and within the community (approximately 50/50 split). You will be responsible for managing a portfolio of community-based elderly customers with Home Care Packages.

Your primary responsibilities will include assessing the needs of your clients, developing personalized care plans, and managing budgets to ensure each client's unique requirements are met. You will also be involved in signing up new customers and proactively managing existing ones while maintaining a person-centered approach.

Collaboration and coordination with Nurses and Allied Health professionals will be crucial to delivering comprehensive care. Your role will require strong communication skills, both verbal and written, as well as demonstrated empathy and compassion.

In return for your expertise and dedication, we offer a competitive salary of $85,000 - $95,000 per annum, depending on experience. As a valued member of our team, you will also enjoy a range of benefits including professional development opportunities and a supportive work environment.
