Occupational Therapist Grade 2- Transition Care Program

  • Position located at Heidelberg Repatriation hospital, working with clients at our partner Aged Care Facility and in the community.

About the Transition and Restorative Care Programs

The Transition and Restorative Care Programs sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The programs are committed to providing high quality safe care for patients while helping them achieve their goals. The programs are provided in both residential and community settings.

About the Role:

The Grade 2 Occupational Therapist (OT) casual will provide Occupational Therapy leave coverage to our Community Aged Care Programs. This includes our Transition Care and Restorative Care Clients. OT Services may be provided in either community or residential settings.

The Grade 2 Occupational Therapist will work closely with the wider Transition Care Team (including medical staff, case managers, allied health and external service providers) to provide a comprehensive service to clients.

A regular day will see you:

  • Providing high quality patient-centred assessments of clients occupational performance and subsequently determining goals and treatment strategies 
  • Actively participating in case conferences and family meetings as required
  • Working closely with the wider TCP team to ensure the delivery of safe and effective client care and supporting d/c planning

About you

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.

You'll further have:

  • Current registration with AHPRA as an Occupational Therapist 
  • Strong clinical skills and demonstrated experience in an aged care or community setting
  • Excellent interpersonal and communication skills that support patient care, team engagement and collaboration
  • A willingness to think outside the square and think creatively to achieve the best possible client outcomes
