Key Responsibilities- Deliver person-centred assessments, interventions, and complex discharge planning to achieve positive patient outcomes
- Work collaboratively with a multi-disciplinary team to achieve shared goals and objectives
- Develop and maintain effective relationships with patients, families, and healthcare professionals
- Contribute to the development and implementation of evidence-based practice and quality improvement initiatives
- Developed clinical experience in a public hospital setting
- Passion for delivering high-quality patient care and contributing to a collaborative team environment
- Effective self-management skills and ability to work in a dynamic environment
