HOPE has been a provider of community services in Western Australia for over 120 years, providing much needed support for people in the areas of alcohol and other drugs, mental health, justice and family and domestic violence.  

About the Opportunity 

ITC Coordinator is a qualified health worker (e.g., nurses, Aboriginal Health Workers) who support eligible Aboriginal or Torres Strait Islander people to access the services they need to treat their chronic disease according to their General Practitioner (GP) care plan.

You will work closely with the client’s GP and assist clients to access a range of services such as appointments with specialists and allied health professionals and make arrangements for home help and connections with support groups. Information on the services the client has engaged with is fed back to the GP for inclusion in the client’s care plan so that it can be considered in future reviews of the plan.

The work of an ITC Coordinator can include providing clinical care, arranging the services in client care plans and assisting clients to participate in regular reviews by their primary care providers. ITC Coordinators can provide support to identify when a client’s condition may require further assistance from a health professional.

About you:

  • Commitment to upholding HOPE’s vision, mission and values.
  • Experience working with Aboriginal and Torres Strait Islander people.
  • Qualifications in nursing or Aboriginal health.
  • Demonstrated ability to work collaboratively with a range of health professionals, including specialists, GPs, nurses and allied health professionals.
  • High level of written communication skills, including report writing.
  • An understanding of and adherence to working within personal and professional boundaries.
  • Willingness to drive across the Goldfields Region.

Desirable criteria:

  • Good understanding of the local health system, including referral pathways.
  • Current knowledge of and commitment to Equal Opportunity in all aspects of employment and service delivery.
  • Aboriginal and Torres Strait Islander heritage.

Appointments will be subject to Australian residency/visa, satisfactory National Police check (within 3 months), Working With Children card, Current C or A class driver's license, and suitable work referees.
