Our client is a leading Aged Care Provider of In-Home Care that enables older people to stay at home and maintain their independence. They have been offering truly heartfelt services to seniors for over 25 years now and expanding.

We are looking for a driven and motivated Care Coordinator to liaise with a portfolio of clients and Care-givers, coordinating the provision of in-home care to enable clients to live safely, comfortably, at home. You will be the type of person that can develop relationships with clients from the very first phone call and really make a difference in their lives.

Duties are but not limited to:

• Growing and maintaining relationships to provide excellent in-home care to our clients.
• Manage an elderly client's total care needs, WH&S, rostering, and complete care plans.
• Assessing and resolving problematic developments and situations
• Email/Phone/Inbox communication with Care-givers and clients.
• Liaise, strengthen, and develop relationships with clients and care-givers.

Skills and experience:

• At least 2 years' experience working in an office environment with an Aged Care provider.
• Experience with Microsoft Office Applications
• Excellent communication skills verbal and written.
• Superior time management and attention to detail.
• Adaptable with the ability to handle competing tasks.
• Vaccinated with Covid-19 vaccine.
