This is a full time role offered on a fixed term contract (with the possibility of extension based on funding). Your journey with Feros Care will commence with a comprehensive training and orientation session to support you in your new role.

**About the role**:

- Help your clients to remain at the centre of their journey
- Develop **HPC/STRC **care plans and coordinate a multi-disciplinary team (MDT) for the delivery of in-home therapy, services, and equipment
- Manage & Coordinate the day to day delivery of care for **HPC/STRC **clients including managing their individualized budgets.
- Be the advocate for the client and participate in case conferencing and scheduled clinical appointments as required.

**About you**:

- **Tertiary qualifications in Nursing or Allied Health (with relevant registration) Desirable**:

- ** Previous Case Management Experience (Community and Aged Care experience highly desired)**:

- Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
- Current drivers licence and be comfortable traveling throughout the region each day
- COVID-19 Vaccination
