
Transition And Post Care Support Officer, Families, Seniors, Disability Services And Child Safety

As a Transition and Post Care Support Officer you will:
Provide specialist transition from care support that identifies and appropriately responds to the complex support needs of high-risk young people who are at risk of homelessness based on the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle.
Promote and facilitate involvement of the young people in goal setting and planning for their transition while supporting their connection to family, culture and community.
Organise the provision of specialist services that address high risk and complex needs of young people and assist with the development of young people's independent living skills with a central focus on enhancing capacity to maintain safe and stable housing aid their wellbeing and post-care outcomes and reducing their risk of homelessness.
Consult and develop relationships with government and non-government agencies including Queensland Health, Youth Justice, Office of the Public Guardian, Department of Housing and Public Works, and the National Disability Insurance Agency to ensure efficient, effective and financially supported service delivery.
Collaborate with regional line manager and senior program officer within a hub and spoke model ensuring continuous quality improvement and alignment with regional and programmatic priorities and identify opportunities for outcome improvement when supporting young people transitioning from care.
Use specialist transition practice knowledge to contribute to improvements in service delivery and enhance workforce capacity in child safety and external agencies.
Maintain sound, accountable and legislatively compliant record keeping relevant to service delivery including contribution to programmatic data reporting systems.
