We work in partnership with you to help you stay independent at home, achieve personalised goals, or access specialist disability accommodation.The OpportunityWe have an exciting opportunity for a Disability Services Manager – Residential Services to join a supportive and inclusive team.
This role provides client services management for our clients, including care plan management and review, client documentation management, acting as the point of contact for clients and management, and improvement of the client experience through engagement of care teams and stakeholders, and oversight of funding utilisation.The Disability Services Manager – Residential Services will work in partnership with clients and their families to identify assessment care needs and support services required to promote enablement, dignity, independence and choice.
While the primary site for this role is based in Ipswich, a crucial part of the role will involve regularly travelling to and managing SIL/Accommodation homes across Beaudesert and Helensvale.
ResponsibilitiesEngage in client relationship management to ensure a high-quality customer experienceEnsure effective care plan review and managementLead and coach support workers in delivering and exceeding client expectationsMonitor service levels, including review of funding and client/carer satisfactionEnsure consumer support plans and assessments are completed within required timeframesConduct case conferences with internal and external stakeholders with the consumer and/or their representative when needs change or reviews are requiredTo be successful in this role you will haveDemonstrated experience in a Community Services Co-ordination or Case Management roleDemonstrated experience in managing SIL/Disability accommodationsCertificate IV in community services, or relevant discipline (Preferred)Demonstrated experience supervising, supporting, and training staffKnowledge of OH&S legislation relevant to aged care and disability service sectorAbility to work effectively as part of a multidisciplinary care teamBuild strong relationships with customers, case managers and other key stakeholdersHigh level of written and verbal communication skillsAdditional RequirementsNDIS Worker Screening CheckNational Police CertificateCurrent Driver's licenseWhat's in it for youOpportunity to work for the nation's largest health and aged care provider, with a rapidly growing and progressive visionFriendly team environment with a strong community care spiritEmployee Assistance Program for staff and their familiesRostered day off for real work-life balanceHow to applyTo apply select the 'apply' button and you will be redirected to our website to commence your application.Note: Candidates may be shortlisted and interviewed prior to the closing date; apply today.Equal OpportunityClaro prides itself on being an equal opportunity employer.
