The St Vincent de Paul Society is a leading provider of community support services, whose values it is to shape a more just and compassionate society.

**The role**:
Vital and diverse role providing a range of **Case Work** support to people experiencing homelessness. The support you provide will be crucial in helping people stabilise and sustain their accommodation and ultimately transition away from homelessness. This is a highly rewarding and diverse role with a mixture of outreach, tenancy and crisis case management support.

**Your responsibilities**:

- Engaging with individuals sleeping rough or at risk of homelessness
- Conducting home visits with clients in temporary, transitional, or permanent accommodation
- Referring clients to appropriate external services and linking them to life skill and educational programs
- Building collaborative partnerships with key stakeholders and community service providers
- Offering non-judgmental, respectful, and inclusive support to clients, upholding their rights and maintaining confidentiality
- Developing and monitoring case plans to support clients in achieving their goals
- Attending to the primary needs of clients and maintaining accurate client records and case notes
- Meeting service targets set by the manager, Society, and external agencies

**To be successful, you will need**:

- Previous experience providing support to marginalised and vulnerable individuals
- Knowledge of issues related to homelessness, including family breakdown, mental illness, and addiction
- Awareness of local community support services for homeless clients and the ability to build strong relationships with service providers
- Excellent written and oral communication skills
- Proficiency in computer skills
- A valid NSW driver's license
