
Case Manager Position

We are seeking a skilled and compassionate Support Worker to join our team in providing case work support to individuals experiencing homelessness.
The ideal candidate will have excellent communication skills, a non-judgmental attitude, and the ability to work collaboratively with clients, stakeholders, and internal teams.
Responsibilities:
Engage with persons at risk of or experiencing homelessness, undertaking home visits and referrals to external services as required.
Develop and implement collaborative partnerships with other key stakeholders and services.
Provide client support with respect, empathy, and confidentiality, while upholding client rights at all times.
Develop and review case plans with clients, providing ongoing support to achieve desired outcomes.
Maintain accurate records of client interactions, monitoring progress and reporting as required.
Meet service targets and contribute to a positive team environment through active participation and collaboration.
