Join a purpose-led team delivering exceptional in-home care across the Northern Rivers region.Right at Home – Northern Rivers is seeking a Full-Time Registered Nurse – Clinical Care Coordinator to join our dedicated and growing team in Ballina.
This pivotal leadership role combines clinical nursing, care coordination, and relationship management to support elderly, frail, and disabled clients with person-centred, high-quality care.About the RoleAs a Clinical Care Coordinator, you'll be instrumental in delivering consumer-directed services that promote independence, dignity, and wellbeing.
Your day-to-day will include:Conducting clinical assessments and developing tailored care plansOverseeing the coordination of an allocated case load of client services and supporting care staffProviding direct nursing care when requiredIdentifying and managing clinical risks with a proactive approachOffering training and support to team membersMaintaining compliance with aged care legislation and quality frameworksDemonstrating excellent communication and empathy in client interactionsYour Key ResponsibilitiesClient-Focused Care:Conduct in-home assessments and prepare individualised care plansProvide clinical education and support to clients and their familiesMonitor care quality and implement nursing interventions as neededRespond to and manage service enquiries and referralsBuild strong relationships with external providers, referrers, and community stakeholdersWhat We're Looking ForAHPRA Registration as a Registered Nurse (essential)Demonstrated leadership and coordination experience in aged or community careStrong knowledge of aged care legislation, WHS, and quality standards an advantageExcellent interpersonal, problem-solving, and documentation skillsComfortable using clinical and scheduling software (VisualCare or similar)A genuine passion for person-centred care and continuous improvementCurrent AHPRA Registered Nurse certificationNational Police ClearanceAustralian Driver's Licence and reliable, insured vehicleStrong verbal and written communication skillsKnowledge of aged care frameworks, including the Aged Care Quality StandardsDesirable QualificationsPostgraduate certification in geriatric nursing, dementia care, or palliative careTertiary qualifications in health, aged care, or managementWhy Join Right at Home?
