About Our Home Care Service

We are seeking an experienced Clinical Care Coordinator to join our team, providing exceptional in-home care services to elderly, frail, and disabled clients.

Your Key Responsibilities
  • Conduct comprehensive assessments of clients' needs and develop tailored care plans to promote independence, dignity, and wellbeing.
  • Provide direct nursing care when required and ensure compliance with aged care legislation and quality frameworks.
  • Identify and mitigate clinical risks through proactive approaches and demonstrate excellent communication and empathy in client interactions.
Requirements
  • AHPRA Registration as a Registered Nurse is essential.
  • Demonstrated leadership and coordination experience in aged or community care is highly valued.
  • Familiarity with aged care legislation, WHS, and quality standards is advantageous.
