Purpose of Position

The Alcohol & Other Drugs Counsellor Case Manager (AOD) Counsellor Case Manager will deliver integrated individual counselling/case management and intensive day rehabilitation group programs. This will include group facilitation in addition to case management of an independent caseload and the provision of specialist assessment on the level of drug dependence.

**Responsibilities**:
Provision of Treatment and Education Services
- Provide high quality engagement, assessment, counselling, case management and support services to clients, and their families, using evidence-based practice principles
- Facilitate intensive group therapy day rehabilitation programs with other co-facilitators
- Manage a case load of clients, through providing support, service coordination, referral and advocacy to assist client in achieving their goals.
- Work with clients to develop, implement and review individual treatment goals
- Ensure continuity of care by effectively communicating within the multidisciplinary team and across interfaces with other health care providers

- Engage with and develop productive relationships with relevant stakeholders
- Provide culturally appropriate practice as relevant to varying clients
- Ensure that client records, data collection and reporting are consistent with confidentiality and relevant practice standards
- Articulate and discuss AOD and its associated factors with the team to align service delivery with evidence-based practice
- Actively ensure all procedures and relevant systems are implemented, understood and adhered to on an ongoing basis

Position Description

Responsibility:

- Maintain file records, data collection processes, outcome measures and reporting as required
- Ensure that records, data collection and reporting are consistent with confidentiality and relevant practice standards
- Complete all relevant documentation in a timely manner and with clarity, wherein the team can review and conceptualise data relating to individuals and families based on the quality of documents completed
- Deliver treatment and report monthly on the treatment services provided. For example, this includes keeping records of the treatment provided and rationale for its use, the location, the number of clients and ATSI serviced per week and the number of referrals per week

Program Management
- Develop clinical practice knowledge and expertise through active learning within the team and by engaging in professional development activities
- Participate in, and contribute to, regular case conferences, presentations and team discussions in relation to clinical practice and issues
- Actively engage in, and contribute to the development of the Community Services team
- Implement knowledge of social and psychological functions, as interacting with AOD use, into practice
- Articulate and discuss AOD and its associated factors with the team to align service delivery with evidence-based practice

Quality Assurance
- Actively ensure all procedures and relevant systems are implemented, understood and adhered to on an ongoing basis
- Conduct practice in a manner that is consistent with professional standards, ethical principles and the code of conduct

Client Relations
- Provide culturally appropriate practice as relevant to varying clients. This includes engaging in ethical practice by identifying and meeting the social and cultural needs of consumers
- Ensure that services provide respect and protect the rights of consumers and carers and maintain their privacy and confidentiality
- Respect the cultural diversity amongst individuals and families and uphold ethical standards in line with both Lives Lived Well and the relevant profession
- Emphasise the importance of engagement in building rapport and working relationships with clients and their families
- Collect and monitor feedback data from consumers by ensuring consumer views are taken into consideration during service delivery. Ensure that improvements are made where appropriate
- Ensure barriers, including any structural and communication barriers, are addressed

Teamwork
- Attend and actively participate in team meetings and project work, including liaising with other LLW services within the Moreton Bay community
- Communicate effectively and openly in the workplace
