Home Care Coordinator
- Coordinate services for HCP clients in accordance with their care plan and as agreed on budget
- Liaise with third party contractors including allied health professionals, gardeners, external agencies, etc. to organize requested services for clients
- Supervise staff and monitor reporting
- Roster for all scheduled services
- Carry out all administrative duties including proper documentation
- Organise quotations and trials of approved aids and equipment for clients and liaise with suppliers for the purchase and delivery
- Ensure all invoices are redirected to manager and accounts for payment on a timely manner
- Attend all calls for homecare from new, potential and existing clients
- Be the main point of contact for client's queries and grievances
- Be the main point of contact for external parties and companies
- Complete monthly reporting requirements
- Attend new referrals on site and try to expand the number of clients
- Occasionally attend existing clients that may need additional support
- Use existing tools to gather feedback from clients formally as well as informally
- Gather feedback from staffs formally and informally
- Organize and attend meetings
- Coordinate with manager for compliance requirements
- Assist manager for financial submission
- Carry out ad hoc duties as necessary

To be considered for the role you must have the following:

- Car
- Drivers License

**Job Type**: Contract
Contract length: 12 months

Pay: $28.00 - $40.00 per hour

Expected hours: 38 per week

Schedule:

- Day shift
