The NDIS Support Coordinator ensures the planning and delivery of a flexible, integrated, and timely service for NDIS participants. In this position you will have a sound knowledge of funded and unfunded supports to oversee support coordination in accordance with participants NDIS plans. Our team work collaboratively with a strong alignment to Flagstaff's core values. Our Support Coordinators are passionate, experienced and are committed to providing a person-centred approach.

**About the role**:
Support Coordinators are the link between NDIS participants and quality services and supports.

**As a Support Coordinator, you will**:

- Develop & maintain effective collaborative relationships with NDIS participants, families, carers, advocates, and other service providers
- Link NDIS participants with services, supports and programs according to their individual needs and goals
- Review and monitor NDIS plan outcomes and expenditure in conjunction with plan managers to ensure the plan remains in budget
- Provide clear, accurate record keeping of case notes, information, and billing
- Support NDIS participants with the plan review process
- Develop and foster strong networks with the local community

**As the successful applicant, you will possess the following Essential Criteria**:

- Minimum 12 months experience in a Support Coordination role, LAC, NDIS planner or similar.
- Experience working with people with a disability living within complex situations;
- Demonstrated experience in interpreting and implementing individual NDIS plans;
- Experience working with people living with a disability who may require additional support with their communication or cultural needs;
- High level of verbal & written communication, interpersonal and negotiation skills;
- Proven experience in record keeping including an ability to maintain confidentiality and security of information;
- Minimum Cert IV in Disability or other qualifications relevant to the field;
- Drivers licence or ability to provide transport to NDIS participants;
