**Your Role**:
Join a passionate team where support, collaboration, and career development are key. As a Care Coordinator, you'll make a real difference by providing high-level, person-centered care to clients with complex needs, including substance use issues. You'll be a key player in delivering the **AOD Continuing and Coordinated Care Program (CCCP)**, ensuring clients receive specialist care and support for the best outcomes.

**Your Responsibility**:

- Conduct comprehensive assessments and co-create tailored care plans with clients
- Collaborate with referral partners and agencies to provide holistic, wrap-around support
- Establish and maintain referral pathways to connect clients with essential health and community services
- Ensure culturally appropriate care for Indigenous Australians, linking them with relevant services
- Maintain accurate client records, case notes, and data
- Offer direct support in areas such as employment, housing, education, daily living skills, and community reconnection

**You Will Need**:

- Relevant tertiary qualifications and significant experience in care coordination, particularly in **AOD** and **mental health**:

- Proven ability to conduct assessments using standardized tools
- Experience providing therapeutic interventions and working with clients in a non-judgmental, ethical manner
- Familiarity with community resources and the ability to work independently and in a team
- Alignment with the CCCP, the Society's AOD Model of Care, and Client-Centered Service Delivery
- Strong computer literacy, especially in data collection
- A valid NSW driver's license
