Home Care Coordinator Job Opportunity

We are seeking a dedicated and compassionate Home Care Coordinator to join our team. As a key member of our team, you will play a pivotal role in planning, coordinating, and monitoring care and support services for clients.

About the Role

The Home Care Coordinator is responsible for managing incoming referrals, conducting client assessments, developing individualized care plans, coordinating staff schedules, and monitoring service delivery to maintain quality and client satisfaction.

Key Responsibilities
  • Manage incoming referrals and conduct thorough client assessments
  • Develop personalized care plans tailored to client goals and needs
  • Coordinate staff schedules and match carers with clients efficiently
  • Monitor service delivery to ensure high-quality outcomes and client satisfaction
  • Maintain accurate client records and adhere to relevant policies and procedures
Requirements
  • Certificate III in Aged Care or Individual Support (or equivalent)
  • Current First Aid Certificate
  • Valid Australian Driver's Licence and access to a reliable vehicle
  • Current Police Check and NDIS Worker Screening Check
  • Experience working with home care packages and a strong understanding of the Aged Care Quality Standards
  • Excellent interpersonal skills and ability to build strong relationships with clients, families, and staff
