**Description**:
Right at Home provide exceptional in-home care and support for a range of clients in the **Perth South-Eastern Suburbs** **"Our Mission is to improve the quality of life for those we serve".**

We are seeking a Relationship Officer who will be the bridge between Right at Home and the community, identifying gaps in services, gaining referrals, and building strong connections with various stakeholders with the aim of develop and maintain new and existing referral sources.

This is a Client's centered role focused in quality outcomes achieved using a flexible, responsive, creative and collaborative approach.

**Qualifications**:
The Relationship Officer will be responsible for:

- Proactively engage with local communities to identify gaps in services and resources available to our clients
- Build and maintain relationships with community organisations, businesses, and service providers to enhance our referral network
- Oversee and coordinate the onboarding process of new Home Care Package clients
- Organising support to clients, monitoring and reporting on their progress
- Assist clients in identifying ongoing needs and goals to support them to remain in their own home

**Preferred Skills**:

- Experience in community engagement, networking, marketing, outreach, sales, or similar roles, preferably within the aged care sector.
- Knowledge of Case Management principles, Home Care Package Guidelines and Funding body.
- A strong client focus which is based on their goals, preferences and needs
- Ability to work within a team, as well as autonomously
- Excellent organisational skills and ability to manage multiple priorities
- Excellent verbal and written communication skills and strong computer skills and experience with client database systems
- Current National Police Check
- Full Drivers License
- Right to work in Australia
