We are looking for a Clinical Case Manager to join our expanding team. Specialising in working with people living with a disability, the successful applicant will contribute as a key member of our multi-disciplinary team.
SACARE is a South Australian-owned and operated organisation leading the way in providing innovative in-home and supported accommodation care solutions to a diverse range of participants. Here at SACARE, we are seeking a full time Clinical Case Manager.
- Ensures that client health and well-being, capabilities, independence, choice, privacy, dignity, and safety are maintained and enhanced through the provision of quality clinical care plan development and review
- Appropriate assessment & development of care plans for new clients including those from Complex Care Pathway care plans
- Support individuals to formulate goals and aspirations within the Peace of Mind Framework
- Maintains a current knowledge and best practice in clinical care and lifestyle service delivery.
- Strives to improve the quality of service provided within SACARE.
- Acts as a positive role model to nursing and support staff.
- Ensures that all incidents are reported promptly and accurately with the appropriate risk assessment procedures undertaken
- Ensures that all relevant legislative requirements relating to care delivery are always complied with and ensures facility staff have awareness of those requirements.
- Maintain a current knowledge of the NDIS High Intensity Daily Personal Activities requirements
- Liaise with the General Manager -Accommodation Services and Clinical Nurse-Learning & Development as it relates to decisions affecting client care & clinical needs.
- Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Manual handling certificate
- First Aid certificate
- NDIS Workers Check
- Working with Children's Check
- Clinical expertise in the management of medical conditions related to disability care
- Experience working in disability care
- Excellent administrative, time management and organisational skills
- Demonstrated positive and flexible attitude with a willingness to take on a wide range of diverse tasks and adjust to competing priorities.
- Excellent interpersonal and communication skills with the ability to work with individuals from all socio-economic and cultural backgrounds;
- Sound ability to use the Microsoft Office suite of products.
