About the role
Case manager for Home Care Package Consumers
Full time position
Office location - Warabrook
**Your main duties will include**:
Delivery of case management that is person centred and focused on enablement.
Utilising evidence based practice to deliver quality care and service
Develop positive working relationships with clients and within the organisation to facilitate improved outcomes for clients.
About you
Experience with case management for Home Care Package consumers
Minimum qualification - Diploma in community services
Knowledge of various funding sources (i.e. Commonwealth Home Support Program, Respite Care and Community Nursing)
Positive attitude and can do approach for tasks
