Role Summary
Mental Health Community Worker are experienced community workers whose roles have an emphasis on working with clients with psychosocial support requirements. The role works within our Mainstream Capacity Building (MCB) team and aims to reduce the readmission cycle to hospital for clients, as well as focusing on barrier minimisation strategies for people living with Disability.
Responsibilities
Assisting clients to live independently and to access supports and services
Developing support plans based on the needs of individual clients
Monitoring the progress and providing reporting on support plans
Liaising with the families and representatives of clients
Seeking feedback and ensuring this is used to improve services
Networking and connection to services
Skills, Attributes And Experience
Our ideal candidate will have previous experience working in the mental health sector or with clients requiring significant psychosocial support or have recently completed qualifications in a mental health field. They will be proactive, positive and communicative, with a commitment to providing quality support services and building meaningful, lasting relationships based on trust and respect. They will be a proficient problem-solver who remains confident in difficult situations and works collaboratively to achieve positive outcomes.
Key Requirements
Cert IV or higher qualification in a relevant field
Possess or be willing to obtain a NDIS Worker Screening Check
Hold a valid Driver's Licence and a reliable vehicle
Previous experience working within mental health is highly desirable
