Job Opportunity

We are seeking a dedicated and passionate individual to fill the role of Mental Health Community Worker. This position involves working with clients who require psychosocial support, focusing on reducing hospital readmissions and minimizing barriers for people living with Disability.


Responsibilities
  • Supporting clients to live independently and accessing necessary services and resources
  • Developing tailored support plans based on individual client needs
  • Monitoring progress and reporting on support plan effectiveness
  • Collaborating with families and representatives of clients
  • Encouraging feedback and utilizing it to improve services
  • Networking and connecting clients with relevant services

Requirements
  • Certification in a relevant field or higher qualification
  • Possession of or willingness to obtain a NDIS Worker Screening Check
  • Valid Driver's Licence and reliable vehicle
  • Previous experience working in mental health is highly desirable
