You may encounter new challenges, technologies, and best practices in different settings, which can enhance your clinical skills and knowledge.Main Duties:Responsible for supporting referrals for the area you are assigned to, carrying-out comprehensive assessments and developing perinatal specialist mental health care plans.They will monitor and support women during the pregnancy or at post-natal stages, maintain strong links and communication with health providers such as Community Midwives, Specialist Midwives, Health Visitors and other professionals.Planning care programme approach (CPA) reviews.Required to attend and cover key functions in the team,- duty, team huddles, team meeting, buddy support/lone working.As a member of a large team, you will provide support to your colleagues with complex cases, escalating concerns to Safeguarding partners and ensuring that safety plans are in place for high-risk service users.To keep up to date with specific specialist perinatal research.Undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.Managing a caseload and the provision of mental health care services to people with mental health care need.To be responsible for accurate recording of mental health records of all patients on admission, discharge and at review.To work within the multi-disciplinary team and to form working partnerships with other care providers.To ensure service users and / or carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the multi-disciplinary team.Requirements of the Social Worker:Degree or equivalent in social workCurrent Social Work England registrationSignificant frontline experience in Adults Services within the UKContact:This Social Worker job is advertised by Rihana Chowdhury.
