Applications are sought from suitably qualified Allied Health Professionals for the role of Emergency Department and Complex Admissions Care Coordinator. This role is based in the Emergency Department and working within the Wellbeing and Primary Mental Health team. This pivotal position leads care coordination across ED presentations and complex inpatient admissions, ensuring timely access to integrated hospital and community services that optimise patient outcomes.
Key responsibilities include:
Deliver quality, patient-focused care through competent, evidence-based assessment, intervention and discharge planning
Identify and manage risk, implementing risk-reduction strategies for presentations involving mental health, family violence, alcohol and other drug use and child abuse
Promote continuity of care by developing comprehensive discharge plans and ensuring effective communication with the treating medical team, including Primary Care providers
Facilitate seamless transitions between hospital and community services to support optimal patient outcomes
Actively participate in mandatory meetings as required and requested
Maintain core clinical competence specific to the area of clinical practice
Qualifications/experience required:
Tertiary qualification in Social Work, Occupational Therapy or related discipline
Minimum 7 years' experience
Eligibility for registration with peak body
